At Renacer Foundation, our Community Health Workers (CHWs) are a vital part of our community impact. In addition to our institutional team, we have a specialized group of CHWs who work directly in the community, providing close, compassionate support to patients and families.

Our CHWs assist individuals in navigating the healthcare system, accessing community resources, and addressing social and practical needs during recovery and wellness processes. Through personalized guidance and consistent follow-up, they help strengthen continuity of care and promote overall well-being with a human, culturally sensitive, and community-centered approach.
