Our Community Health Workers (CHW) Team
At Renacer Foundation, our Community Health Workers (CHWs) are a vital part of our community impact. In addition to our institutional team, we have a specialized group of CHWs who work directly in the community, providing close, compassionate support to patients and families.
Our CHWs assist individuals in navigating the healthcare system, accessing community resources, and addressing social and practical needs during recovery and wellness processes. Through personalized guidance and consistent follow-up, they help strengthen continuity of care and promote overall well-being with a human, culturally sensitive, and community-centered approach.

Diego Chavez
Diego oversees the SRA program at Renacer Foundation. He coordinates daily case management, guides the CHW team, and maintains communication with SRA and partners, focusing on quality documentation, compliance, and continuous process improvement.

Pedro supports SRA program supervision by coordinating services such as transportation and logistics. He assists CHWs with complex cases, reviews progress, and promotes clear documentation, standing out for his practical approach and teamwork.
