ORGANIZATIONAL SCOPE Renacer en Vida Nueva, Inc. ("Renacer") is a faith-based 501(c)(3) nonprofit community health organization. Renacer's services are limited to health education, service referral, and benefits navigation. Renacer does not provide clinical treatment, process health insurance claims, or engage in electronic health care transactions that would classify it as a covered entity under the Health Insurance Portability and Accountability Act of 1996 (HIPAA).


VOLUNTARY COMMITMENT Notwithstanding the above, Renacer voluntarily adopts the standards of the HIPAA Privacy Rule (45 CFR Parts 160 and 164) as the governing framework for the handling of any individually identifiable health-related information received or maintained in the course of its work. This commitment reflects Renacer's ethical obligation to the communities it serves and its recognition that individuals who share personal health information in connection with navigation and referral services deserve the same standard of care afforded to patients of covered entities.


WHAT WE PROTECT In the course of providing health education, service referral, and navigation assistance, Renacer may receive individually identifiable information relating to an individual's health status, social determinants of health, insurance status, or health service needs. Renacer treats all such information as confidential and subject to the protections described in this Statement.


HOW WE HANDLE HEALTH INFORMATION Renacer staff and volunteers are trained to handle health-related information with discretion and professionalism. Specifically, Renacer:


  • Collects only the minimum information necessary to connect an individual to appropriate services or resources
  • Does not disclose individually identifiable health information to third parties without the individual's express consent, except as required by law
  • Does not sell, rent, or share health-related information for commercial purposes
  • Maintains physical, administrative, and procedural safeguards to protect information from unauthorized access or disclosure
  • Retains health-related records in a secure manner consistent with applicable state and federal guidance


INDIVIDUAL RIGHTS Consistent with HIPAA Privacy Rule standards, Renacer honors the following rights for individuals whose information it holds:


  • The right to know what information Renacer maintains about them
  • The right to request correction of inaccurate information
  • The right to request that Renacer limit how their information is used or shared
  • The right to withdraw consent for any non-essential use of their information at any time


COMPLAINTS Individuals who believe their health-related information has been improperly used or disclosed by Renacer may contact the Privacy Official below. Renacer will not retaliate against any individual for raising a concern in good faith.
