Community Health Initiatives
Renacer is a recognized and proven subject matter expert in nonclinical community health, delivering a core set of community health priorities: access to healthcare, behavioral and mental health, chronic disease prevention, senior care, and HIV care continuity.
Our Florida Certification Board authorization as a Sole Source Provider makes us the only FCB-approved CHW curriculum in the region with a dedicated mental wellness track in addition to a physical wellness track.
We operate Community Health Initiatives, which houses our community health work. It is built on our Florida Certification Board authorization as a Sole Source Provider (#5580-A, certified January 27, 2026), the only FCB-approved CHW curriculum in the region with a dedicated mental wellness track in addition to a physical wellbeing track, both with foundations in economic development. The initiative contains five elements: the Community Health Practice program, the PUENTES CHW training curriculum, our CHW workforce, the Bienestar Mental track, and the Welcome Home project.


Community Health Practice
Renacer delivers CHW-led health navigation, referral coordination, education, and direct in-home support, closing the healthcare access gap in the communities we serve.
Our deployed CHW service model addresses the documented insurance gap: 11.1% of Orange County adults aged 18 to 64 lack health insurance, and we close that gap through bilingual, in-home, nonclinical health navigation.
We operate the Community Health Practice as our deployed CHW service program within the Community Health Initiatives initiative. Through this practice, we deliver nonclinical health navigation, referral coordination, education, and direct in-home support across Orange, Seminole, and Osceola counties.
We established the practice in response to the documented health insurance gap in our community: 11.1% of Orange County adults aged 18 to 64 lack health insurance. We address that gap directly through educational programming, healthcare navigation, and assistance in obtaining insurance. We also operate an elder-care program within the Community Health Practice as a standalone partnership, enrolling older adult residents, obtaining consent, scheduling visits, and delivering bilingual community health navigation and in-home support aligned with residents' needs at two sister senior-housing campuses.
We deliver this work alongside AdventHealth and Senior Resource Alliance (SRA), the primary healthcare system and Area Agency on Aging partners; COAMED (Colombo American Medical Association / COAMED Foundation), which brings a network of Colombian-American physicians and medical professionals to community health missions with a documented history of delivering free medical screenings, consultations, and triage care; and Westminster Communities of Florida, with whom we deliver our elder-care program at Magnolia Towers and Westminster Plaza East.




PUENTES CHW Program
Renacer develops accessible, community-rooted Community Health Workers through the region's only FCB-approved curriculum with dedicated mental wellness and physical wellbeing tracks.
Our PUENTES program holds three identities at once: our internal CHW development pipeline, a CareerSource Central Florida-contracted workforce program, and the only FCB-authorized CHW curriculum in the region, with two distinct certification tracks.
We operate PUENTES as our Community Health Worker training curriculum within the Community Health Initiatives initiative. PUENTES holds three identities at once.
1. Our internal CHW development pipeline. PUENTES is the program through which we develop the bilingual, community-rooted CHW workforce that staffs our own Community Health Practice, including the Welcome Home project for seniors and our CHW-led behavioral health and HIV care work.
2. A CareerSource Central Florida contracted workforce program. PUENTES is one of three CSCF-funded workforce efforts we partner with, alongside our adult workforce development work for Florida's displaced workers and the Summer Youth Program (see Section 6). CareerSource Central Florida is the regional workforce board for Lake, Orange, Osceola, Seminole, and Sumter counties. Through this contract, CSCF funds PUENTES so that the same FCB-approved CHW training is made available to other job seekers and individuals seeking to upskill into one of the fastest-growing community health roles in the country.
Our CareerSource Central Florida partnership culminated in a workforce and community English partnership with CSCF, the City of Orlando, and UCF Global, with a cohort delivered in 2024.
3. Our FCB-authorized curriculum. PUENTES is the curriculum that earned us our Florida Certification Board Sole Source Provider authorization (#5580-A, certified January 27, 2026), delivered in multi-week cohorts. We are the only FCB-approved CHW training provider in the region offering two distinct certification tracks: a mental wellness track (Bienestar Mental) and a physical wellbeing track, both with foundations in economic development.
On the educational side, we have collaborated with UCF Global, together with whom CSCF delivered a workforce and community English cohort in 2024.
Our government partner on this work is CareerSource Central Florida (CSCF), the regional workforce board for Lake, Orange, Osceola, Seminole, and Sumter counties, which funds PUENTES as a contracted workforce program.




Welcome Home
Renacer provides 30 days of in-home support to older adults after hospitalization to prevent avoidable readmissions, at no cost to the senior or their family.
Our 30 days of in-home post-discharge support are delivered to older adults at no cost to the senior or their family.
We operate Welcome Home as our largest project by budget and staff within the Community Health Initiatives initiative. We provide 30 days of in-home support to older adults after hospitalization at no cost to the senior or their family, designed to prevent avoidable readmissions and to ensure that the people most at risk of falling through the cracks make it home and stay home, well. Welcome Home is delivered through our Community Health Workers, developed through the PUENTES training pipeline, across multiple hospital campuses along mid-Florida.
In 2024 and 2025, we served 700 senior households, with growth projected to exceed 1,000 by the end of 2026. We currently employ a team of trained Community Health Workers who provide in-home care and support services to more than 100 seniors at any given time in Central Florida.
Welcome Home services span medical and care coordination, medication and pharmacy support, transportation, meal access, home safety modifications, counseling, companionship, optional spiritual care, regular wellness check-ins to reduce loneliness, and respite for family caregivers. The project is designed for older adults discharged from hospitals in the Orlando area, as well as for families and caregivers who need practical support to organize care at home.
In the past, we have received training from the Florida Department of Elder Affairs and supported other programs alongside the Department. Our CHWs have been trained and certified to deliver comprehensive assessments, including the 701B and GUIDE (Guiding an Improved Dementia Experience), a federal program of the Centers for Medicare & Medicaid Services.


Our work has been recognized through our formal nomination by the Senior Resource Alliance for the Florida Department of Elder Affairs grant presentation. In late 2023, we were a key partner in SRA's Enhanced Home Care for the Elderly Program (eHCE), which provided home modifications to allow resource-limited seniors to age safely and independently; when SRA had more than 1,000 senior appointments through the eHCE program, we mobilized 35 case managers to support the program.
We deliver this work alongside AdventHealth, the healthcare system we support in preventing rehospitalization.
Our government partner on this work is Senior Resource Alliance (SRA), the federally designated Area Agency on Aging for Central Florida, through which all Welcome Home clients are referred.
