WHO WE ARE Renacer en Vida Nueva, Inc. (“Renacer”) is a faith-based 501(c)(3) nonprofit organization and HIPAA-covered entity serving Hispanic/Latino and underserved communities in Central Florida. The Renacer Foundation website is owned and operated by Renacer en Vida Nueva, Inc. We are committed to protecting the privacy, confidentiality, and security of all personal, health-related, and website information we collect and maintain. This Privacy Policy explains how we collect, use, protect, and disclose information obtained through our website, programs, web forms, communications, and community health services. This Privacy Policy is accessible at www.renacer.live/privacy and is linked in the footer of our website and on all web forms where personal information is collected.

INFORMATION WE COLLECT Renacer collects personal information in connection with our community health, care coordination, health education, social services, appointment scheduling, program enrollment, and website operations. This information may include name, surname, mailing address, phone number, email address, date of birth, city of residence, organization or affiliation, program enrollment information, appointment or care coordination information, health-related information, and payment or donation-related information where applicable. This may include Protected Health Information (“PHI”) as defined under HIPAA. We also collect phone numbers for the purpose of SMS opt-in consent where expressly provided.

When you visit the Renacer website, we may automatically collect certain information about your device and browsing activity, including web browser type, IP address, time zone, cookies installed on your device, pages viewed, referring websites or search terms, and interactions with the website. This automatically collected information is used to maintain website functionality, identify potential cases of abuse, improve website performance, and generate statistical information regarding website usage.

HOW WE USE YOUR INFORMATION Information collected is used solely to provide care coordination, health education, program enrollment, appointment scheduling, community health services, social services, website functionality, and responses to inquiries submitted through our website. HOW YOUR OPT-IN INFORMATION IS SHARED No mobile opt-in or text message consent will be shared with third parties or affiliates.

Renacer does not sell, rent, or otherwise share your personal information, PHI, or SMS opt-in data with third parties except as required by law, as necessary to provide services through authorized Business Associates operating under a HIPAA-compliant Business Associate Agreement, or as otherwise permitted by the HIPAA Privacy Rule.

HOW WE PROTECT YOUR INFORMATION All PHI is protected in accordance with the HIPAA Privacy Rule, 45 CFR Parts 160 and 164, the HIPAA Security Rule, the HITECH Act, and applicable Florida privacy and healthcare records laws, including Florida Statutes §§ 456.057 and 395.3025. Communications involving PHI via telephone, fax, SMS, or other channels are governed by internal safeguards requiring staff to use minimum necessary information, verify recipient identity when appropriate, and use only approved secure channels.

Renacer maintains reasonable administrative, technical, and physical safeguards to protect personal information and PHI from unauthorized access, use, disclosure, modification, or destruction. However, no method of transmission over the Internet, wireless network, or electronic communication system can be guaranteed to be completely secure.
