WHO WE ARE Renacer en Vida Nueva, Inc. ("Renacer") is a faith-based 501(c)(3) nonprofit and HIPAA-covered entity serving Hispanic/Latino and underserved communities in Central Florida. We are committed to protecting the privacy of all personal and health-related information we collect and maintain. This Privacy Policy is accessible at www.renacer.live/en/privacy-policy and is linked in the footer of every page of our website and on all web forms where personal information is collected.

INFORMATION WE COLLECT Renacer collects the following personal information in connection with our community health, care coordination, and social services programs: name, mailing address, phone number, email address, date of birth, program enrollment information, and health-related information. This may include Protected Health Information (PHI) as defined under HIPAA. We also collect phone numbers for the purpose of SMS opt-in consent where expressly provided.

HOW WE USE YOUR INFORMATION Information collected is used solely to provide care coordination, health education, program enrollment, appointment scheduling, and community health services. Phone numbers collected for SMS opt-in are used only to send the types of messages to which you have consented. We do not use your information for marketing or commercial purposes.

HOW YOUR OPT-IN INFORMATION IS SHARED No mobile opt-in or text message consent will be shared with third parties or affiliates.

Renacer does not sell, rent, or otherwise share your personal information or SMS opt-in data with third parties except as required by law, or as necessary to provide services through authorized Business Associates operating under a HIPAA-compliant Business Associate Agreement, or as otherwise permitted by the HIPAA Privacy Rule.

HOW WE PROTECT YOUR INFORMATION All PHI is protected in accordance with the HIPAA Privacy Rule (45 CFR Parts 160 & 164), the HIPAA Security Rule, and the HITECH Act. Communications involving PHI via telephone, fax, or SMS are governed by internal safeguards requiring staff to use minimum necessary information, verify recipient identity, and use only approved secure channels.

SMS / TEXT MESSAGING If you consent to receive SMS from Renacer, you agree to receive conversational and informational SMS messages from Renacer en Vida Nueva, Inc., including appointment reminders, care coordination updates, and program information.
