Hospital to Home Transition: Comprehensive Care for Older Adults

For an older adult, returning home after a hospital stay can be an overwhelming challenge. At Renacer Foundation, our mission is to promote health among vulnerable communities through faith-based initiatives. Therefore, in a strategic alliance with Senior Resource Alliance (SRA), we have turned that challenge into a comprehensive care experience through our Home Support Program.

This project, which began its successful execution in July 2025, acts as the bridge that guarantees a safe and dignified transition. Our goal is to allow our seniors to recover their health and regain their sense of wholeness in the place where they feel happiest: their home.

From its launch mid-year through the end of 2025, our commitment translated into tangible results, successfully serving 169 clients. Thanks to this personalized 30-day accompaniment for each person, we achieved:

• Helping clients stay healthy and regain their well-being.

• Significantly reducing hospital readmissions.

• Fostering the independence of our older adults in Central Florida.

The success of this initiative lies in our team of Community Health Workers (CHW). They don't just visit homes; they transform lives by facilitating the following services:

• Medical Management: Setting up doctor appointments and reminders.

• Logistical Support: Getting medications organized and finding rides to appointments.

• Home Wellness: Arranging meal delivery (like Meals on Wheels) and making the home safer with grab bars or ramps.

• Emotional Support: Providing someone to talk to when feeling lonely and supporting family caregivers with respite care.

Looking ahead, we confirm that this program will remain active throughout 2026. We are committed to its continued growth and strengthening to serve a greater number of clients in Central Florida. Our ultimate goal remains significantly contributing to the improvement of the quality of life and dignity of the older adults in our community.
