Supporting Seniors After Hospital Discharge
Renacer, AdventHealth and SRA support seniors during the transition from hospital to home.

Leaving the hospital is an important step in recovery, but for many seniors, returning home can bring additional challenges. Transportation to medical appointments, access to food, household essentials, and coordinating services can all become important needs during the weeks following discharge.
Since July 2025, Renacer Foundation has worked alongside AdventHealth and Senior Resource Alliance (SRA) to help seniors navigate this transition and access practical support after returning home.

Bringing the Work Into View
Much of this work takes place outside the hospital through calls, home visits, transportation coordination, appointments, and direct communication with participants.
AdventHealth’s publication and video provide an opportunity to see how this support works in practice and to hear directly from seniors who have participated in the initiative.
For Renacer Foundation, the feature also recognizes the work of the professionals who have supported hundreds of participants since the organization joined the initiative.
Renacer Foundation will continue working alongside AdventHealth and Senior Resource Alliance to help seniors access practical resources and assistance during the transition from hospital to home.

From Hospital to Home
According to AdventHealth, with the patient’s consent, eligible seniors are referred to Senior Resource Alliance, which identifies their needs and connects them with available resources.
Renacer Foundation then works directly with participants during the 30 days following hospital discharge, helping them access those resources.
Support may include transportation, food assistance, household essentials, help with appointments and services, and connections to emotional or pastoral support when needed. Renacer team members may also accompany participants to appointments or maintain contact during the transition home.
765 Participants Served Since July 2025
Renacer Foundation began participating in the initiative in July 2025.
Since then, 765 participants have received support through the program, with a team of 26 professionals contributing to the coordination and assistance provided to seniors.
Each participant may have different needs after leaving the hospital. Some may require transportation, food assistance, or help accessing services identified through Senior Resource Alliance. Others may need support managing appointments or connecting with available resources.
Renacer Foundation focuses on helping participants move from receiving a referral to accessing the assistance identified for them.
AdventHealth Highlights Participant Experiences
In its August 19 feature, AdventHealth shared the experiences of seniors who received support through the program, including Ofelia Mendoza and Magdalena Tapias.
AdventHealth reported that Ofelia needed assistance with transportation and groceries after leaving the hospital. Senior Resource Alliance connected her with resources, while Renacer Foundation helped her access that support.
The feature also shared Magdalena’s experience receiving assistance related to meals, household tasks, and medical appointments.
These stories show how the program continues beyond the initial referral through direct communication, coordination, and practical assistance.
Henry Mendoza, Chief Operating Officer of Renacer Foundation, described Renacer’s approach in the AdventHealth feature by saying, “We try to be like their grandchildren."


Addressing Practical Needs After Discharge
AdventHealth reports that the initiative was developed in response to findings from its Community Health Needs Assessment, which identified transportation, food insecurity, and access to care among the challenges affecting older adults in Central Florida.
These needs can become especially important after hospitalization. A senior may have a medical appointment but lack transportation, or may need help accessing groceries, household items, or other services.
Each organization has a defined role. AdventHealth identifies eligible patients and begins the referral process. Senior Resource Alliance assesses needs and connects participants with resources. Renacer Foundation works directly with participants during the 30-day period to help them access that support.
A Program Expanding Across Central Florida
According to AdventHealth, the initiative began as a pilot at AdventHealth East Orlando and has expanded within its Central Florida Division.
Senior Resource Alliance currently supports participating patients in Orange, Seminole, and Osceola counties, while AdventHealth has also extended the initiative into Lake County through Elder Options.
AdventHealth reported that 122 patients had been referred through the program at AdventHealth East Orlando, with 108 approved for support at the time of its August 19 publication. These figures are separate from Renacer Foundation’s internal total of 765 participants served since July 2025.
