As part of our institutional capacity building and service optimization, at Renacer Foundation we have solidified the training and credentialing process for our Community Health Workers (CHW). This professional qualification is delivered through our PUENTES curriculum, which is authorized and approved by the Florida Certification Board (FCB) under our Sole Source Provider certification #5580-A.

We execute the implementation and funding of this training program through a workforce contract with CareerSource Central Florida, the regional workforce board responsible for overseeing labor training standards in Orange, Osceola, Seminole, Lake, and Sumter counties. Utilizing instructors validated by the institution, this partnership ensures that our operational staff acquire the regulated technical competencies required to execute our initiatives, with a specific focus on our Welcome Home project.

The PUENTES Program: Our Approach to Technical Training and Innovation

Our PUENTES curriculum stands out in Central Florida as the region's only FCB-approved training program that features two distinct certification tracks rooted in economic development: a physical wellbeing track and our dedicated mental wellness track (Bienestar Mental).

Through this framework, we equip our CHWs with standardized tools across three core areas:

Case Management and Resource Navigation: We implement technical protocols to identify, refer, and track specific public and private assistance services.

Preventive Health Education: We align evidence-based guidelines to assist our beneficiaries in managing chronic conditions.

Specialized Assessment Certifications: We train our staff to deliver comprehensive assessments, including the 701B and the federal GUIDE (Guiding an Improved Dementia Experience) program, in accordance with the Centers for Medicare & Medicaid Services (CMS) guidelines.

Impact on Service Quality within Our Welcome Home Project

The professionalization of our CHW workforce directly enhances the performance measures of the Welcome Home project, our largest initiative by budget and staff within our Community Health Initiatives. Through this project, we provide 30 days of post-discharge, in-home support to older adults at no cost to the senior or their family, specifically designed to prevent avoidable hospital readmissions.

Our Welcome Home services encompass medical and care coordination, pharmacy support, transportation, meal access, home safety modifications, companionship, and respite for family caregivers. By deploying a workforce technically certified by the FCB and trained through CareerSource Central Florida, we guarantee a high-precision, bilingual, nonclinical health navigation model.

We execute this effort within an interagency collaborative framework where the Senior Resource Alliance (SRA)—the federally designated Area Agency on Aging—serves as our primary referral entity, and AdventHealth serves as our strategic healthcare system partner in preventing rehospitalizations.

Our Outlook and Sustainability

By the end of 2026, we project that the Welcome Home project will expand its reach to exceed 1,000 senior households served across the region. Our official credentialing by the Florida Certification Board and our partnership with CareerSource Central Florida validate our operational compliance with state and federal public health standards, securing our sustainable infrastructure to close healthcare access gaps in Central Florida.
